Welcome to the Member Resources page, your central hub for everything you need as a Medinah Shriner. Here, you’ll find essential tools, official documents, event calendars, and helpful information to support your journey in our fraternity. Whether you’re a new member or a long-time Noble, this section is designed to help you stay connected, informed, and engaged with the Medinah temple and the wider Shriners international fraternity.

As we continue to grow and serve our community, this page will be regularly updated with new materials and announcements. Access WebFez

Use WebFez to verify your personal information, payments and Shrine activity. In order to log into WebFez you must first be registered.

Step 1: Register

  • Click on this link to be redirected to WebFez' website

  • Click the Register button on the Login page

  • From the Registration page, choose your Temple from the drop-down list and then type in your Member Number and your First and Last names.

  • Click on the Register button. You will be directed to the 'Create a New Account' page. On this page enter an email address that you will use to log in with, confirm the email address, and then enter and confirm your password.

  • Scroll down to read the WebFez User Agreement and Privacy policy. Agree by clicking the checkbox.

  • After the checkbox is selected, click on Register. This will Register you for WebFez and log you into the system.

Step 2: Login

  • Click on this link to be redirected to WebFez' website

  • Enter your login email address and password in the designated fields.

  • Click 'Log In'

Dues Card Instructions

If you have any issues or do not know what your Shrine ID Number is, please call the office at 630-889-1400 for assistance. You will just need to provide your name.

  • Open your phone’s app store

  • Search for “My FezCard Viewer”

  • Install/download the app & then open

  • To log in, use your Shrine ID Number (NOT your Member Number!)

  • Type in your Last Name

  • Select your Home Temple (Medinah)

  • If you are caught up on all of your dues, you should be able to see your 2024 Medinah Shrine Dues Card

Access our forms

Below you can access all the different forms you will need as a Shriner Member. You have the option to either fill out the form on the website, or print it, fill it out and hand it in at the Medinah Shrine office. The forms are organized into different categories such as Petitions, Clubs, Committees & Units, etc. The first category shows all of the Petition forms. The tabs at the top of the container show the different categories. Simply click on the tab you wish to access and you will be able to see the forms in that category. 

Fill out a form online

Next to the form's name you will see a plus icon (+). Click on this icon to open the form. You will see all the form fields that you need to fill out. Some fields are marked with an asterisk symbol (*). These fields are mandatory and need to be filled in in order to submit the form. Once you are done filling out the form, click the 'Submit' button at the bottom of the form. 

Download and Fill out the forms by hand

The last dropdown in each category is called 'PDF Forms to download'. Click on the plus icon (+) to open this dropdown and see all the forms. Click on the form file you wish to download and you will be redirected to another screen where you can download this file. Please note, you are able to fill out this form electronically, but you must first download it before you are able to do so.

    This is what is expected of all new Shriners

    1. Pay dues when you receive your dues notice.

    2. Attend four Stated Meetings per year.

    3. Be as involved as your ability will permit. The more you put into our fraternity, the more you will receive.

    4. The Fez is the symbol of our temple and philanthropy. It should be worn with pride and respect.

    5. Promote our Fraternity and Philanthropy.

    6. Have Fun and Help our Kids.

    Instructions for Candidates from the Recorder

    Candidates must pay full Initiation Fee, Dues, Hospital Assessment and Per Capita Tax before Initiation.


    Initiation Fee - $150.00 plus Current Years Dues

    Dues, Hospital Assessment and Per Capita Tax for new members are prorated as follows:

    If Petition Initiated in:

    • January, February, March - $150

    • July, August, September - $109

    • April, May, June - $129

    • October, November, December - $206

    $206 pays current pro-rated dues plus the entire next year

    By becoming a Permanent Contributing Member (PCM) of Shriners Hospitals with a one-time $150 contribution, you will eliminate the annual Hospital Assessment from your dues notice. This is 100% tax deductible.

    100 Million Dollar Club

    The achievement of raising $100,000,000 will assure maintenance of the “world’s greatest philanthropy”, and continue on a permanent basis the work of the 22 Shriners Hospitals for Children® in treating and restoring youngsters with orthopedic problems and burn injuries to a normal, useful life. You can be an important member of the 100 Million Dollar Club by sending your check for $100 (or stating that you have provision in your will giving at least $100) payable to the Shriners Hospitals for Children®, to the Recorder’s Office at Medinah Shrine Center.


    Nobles who make a contribution may also purchase a $100 Million Fez Tassel Pin, for a fee of $15.00, at the Medinah Office.

    Royal Order of the Rose

    In addition, when a Noble contributes $100 or more to the Million Dollar Club in the name of a Lady, the Noble not only receives a 100 Million Dollar Club Certificate, the Lady will also receive a Royal Order of the Rose Certificate, as well as a rose pin to wear at all Shrine Functions to commemorate her induction into the “Royal Order of the Rose”. The donation can be designated on behalf of, or in memory of, your wife, mother, sister, daughter, granddaughter, grandmother, aunt, or any other special lady in your life. So Nobles, honor your ladies by changing the lives of the children in our care.
